Supplemental Magnetic Resonance Imaging Plus Mammography Compared to Magnetic Resonance Imaging or Mammography By Extent of Breast Density

New BCSC study finds that undergoing breast magnetic resonance imaging (MRI) does not decrease rates of interval or advanced cancer

In a new Breast Cancer Surveillance Consortium (BCSC) study published in the Journal National Cancer Institute, researchers evaluated whether screening breast MRI plus mammography improves screening outcomes compared to MRI or mammography alone for women with dense versus nondense breasts. Using the large BCSC cohort, the study evaluated 52 237 women aged 40-79 years who underwent 2611 screening MRIs alone and 6518 supplemental MRI plus mammography.

The study found MRI screening with mammography increased rates of screen-detected early stage cancer and false-positive recall and biopsy and ductal carcinoma in situ for women with dense and nondense breasts without a concomitant decrease in advanced or interval cancers. Screening with MRI alone compared to supplemental MRI with mammography was associated with statistically higher screen-detected early stage cancer rates among women with dense breasts but no difference in screening failures, harms, or screen-detected DCIS suggesting MRI alone is as effective as MRI with mammography in women with dense breasts.

Undergoing screening breast MRI with or without mammography does not decrease the risk of screening failures in women with dense breasts on average at intermediate breast cancer risk.
